SHORE VIEW NURSING AND REHABILITATION CENTER

Effective Date: August 5, 2026

Medical Aid in Dying (MAID)

The Facility is committed to providing compassionate, resident-centered end-of-life care while respecting the rights of Residents under New York State Public Health Law Article 28-F (Medical Aid in Dying or “MAID”). The Facility has elected not to participate in MAID, including the prescribing, dispensing, ordering or self-administering of medications intended to end a Resident’s life based upon sincerely held ethical and moral convictions. If an eligible Resident requests MAID, the Facility shall promptly facilitate transfer to a reasonably accessible health care provider that is willing to evaluate and participate in MAID in accordance with New York State law. The Facility shall provide the receiving provider with relevant medical records upon appropriate request and authorization to ensure continuity of care. This approach is consistent with the New York State Department of Health guidance, which permits facilities to decline participation while requiring prompt transfer assistance and expressly prohibits staff from administering the medication. Facility staff shall not administer, assist in administering, or otherwise participate in the administration of medication prescribed for MAID under any circumstances. The Facility shall continue to provide medically appropriate treatment, comfort care, pain management, palliative care, hospice services, or any other care required by applicable law.
