Eligibility
We welcome all candidates who are at least 18 years of age and qualify for nursing home placement, sub-acute care, or rehabilitation. Our admissions team will work with you to confirm your eligibility and ensure that you can benefit from our services. The admissions process requires a completed application, a Patient Review Instrument, and a Screen.
